Blisters end more ultras than any acute injury, and nearly all of them come from the same four ingredients: friction, moisture, heat and repetition. You cannot subtract friction from a hundred-mile day, but you can control moisture, armor the spots that always go first, and catch a hot spot while it is still only a hot spot. The protocol that holds up over a long day is boring and mechanical: prep and lube before the start, pre-tape only the places your own feet have already complained about, change socks on a schedule rather than on feel, and stop for two minutes the moment something starts to burn instead of pushing to the next aid station. Everything below is general foot-care education for healthy feet; practice all of it in training, and take anything involving diabetes, neuropathy, circulation problems, infection or a wound that will not close to a podiatrist or your own clinician rather than to a race-day article.

What actually makes an ultra blister

A blister is a shear injury, not a rubbing injury. The outer layers of skin get dragged sideways against the layers below, the bond between them fails, and fluid fills the gap. Four things drive that process on a long day.

Moisture is the biggest lever you control. Damp skin has far more friction than dry skin, and once it macerates it tears at loads it would shrug off dry. Sweat alone does this over six-plus hours inside a closed shoe. A creek crossing does it in ninety seconds.

Heat comes with the moisture and softens skin further. This is why blisters that never appear in a cool spring 50K show up in the same shoes in August.

Repetition is what separates an ultra from a marathon. At roughly 170 steps per minute, a six-hour 50K is about 30,000 footstrikes per foot (6 hours times 60 minutes times 85 strikes). A 28-hour hundred with about 24 hours of moving time at 160 steps per minute is roughly 115,000 per foot. That is close to four times the shear cycles, on skin that has almost certainly been wet at least once.

Swelling changes the fit underneath all of it. Feet reliably swell over long days, which is why a shoe that felt right at 6 a.m. is a tighter shoe at mile 60, and why the standard ultra advice is to size up roughly half a size over your road fit and to leave lace tension slightly looser than feels natural at the start.

Layer one: skin prep and lube

Trim toenails four or five days before the race, not the night before, and cut them square and short so nothing overhangs on descents. Leave your calluses alone in race week. A thick callus that gets aggressively filed the day before can shear as a plate and lift a deep blister underneath it; if you manage calluses, do it in small passes weeks out.

There are two working schools of thought, and both have finishers behind them. The lubrication school reduces friction directly. The drying school reduces moisture instead, using an antiperspirant on the soles for three to five nights before the race or a light dusting of foot powder on race morning. Pick one and rehearse it. What does not work is combining a heavy powder with a heavy lube, because the paste that results is worse than either alone.

If you lubricate, a thick balm beats a thin cream. A tin of Squirrel's Nut Butter is the drop-bag standard for exactly two reasons: it stays workable when it is cold, and it goes onto damp skin, which is the condition your feet will actually be in at mile 40. Work it between every toe, across the ball of the foot, around the heel cup and along the arch. Reapply at every sock change. Stick-format balms are faster for the standing-up applications on thighs, underarms and the vest strap line, but for feet you are already sitting down, and fingers reach between toes better than a stick does.

Layer two: socks are the cheapest control you have

Toe socks remove toe-on-toe shear, which is where a large share of ultra blisters live: the second toe rubbing the big toe, the little toe folding under. Injinji Trail Midweight is the common choice because the individual toe sleeves solve that problem structurally rather than with tape. The trade-off is that they take a minute to put on with cold hands, so practice the change while tired.

Match thickness to distance and terrain. Thin socks breathe and dry faster but give up cushioning; midweight adds a buffer for long rocky descents at the cost of a slightly tighter fit. Whatever you choose, size it correctly. A sock that is too large wrinkles under the arch and a wrinkle is a blister factory; one that is too small compresses the toes and makes the shear worse.

Never race in a sock model you have not done a long run in, and put a spare pair in every drop bag.

Add low gaiters if your race has sand, scree or duff. A pair of low ankle gaiters costs almost nothing in weight and removes the grit variable entirely, which on a dusty course is the highest-leverage ten dollars in the kit.

Layer three: pre-tape only what you already know

Tape the places your feet have blistered before. Do not blanket-tape a whole foot: every strip has edges, and edges under load create the exact shear you are trying to prevent.

The usual suspects are the back of the heel, the medial and lateral forefoot over the first and fifth metatarsal heads, the pad under the second toe, and the tip of the big toe on long descents.

Technique matters more than the product. Clean and fully dry the skin. Use an adhesive primer such as tincture of benzoin if you have skin that sweats tape off, and let it get tacky before applying. Round the corners of every strip so nothing catches inside a sock. Lay tape flat with zero tension, extend it well past the hot spot onto healthy skin, and smooth out every wrinkle. Rigid strapping tape holds longer than kinesiology tape on a foot; kinesiology tape conforms better around a heel. Both beat a plain bandage, which balls up within a mile.

The sock-change schedule

Change on a plan, not on a feeling, because by the time a foot feels bad the damage is already done. A reasonable default:

  • 50K: zero or one change, usually at the halfway drop bag, plus an unscheduled change after any water crossing.
  • 100K: one to two scheduled changes.
  • 100 miles: three or more, typically at the night-transition bag, one deep-night station and the sunrise bag, plus any wet section.

The change itself is a sit-down job and takes three to five minutes done properly: shoe off, sock off, dry the foot with a bandana or small microfiber towel, inspect between every toe, re-lube, fresh sock, shoe back on with lace tension reset for the foot you have now rather than the one you started with. Five minutes at mile 50 is cheap. An hour of walking from mile 70 is not. Sequence the change into your stop the way the 90-second aid-station routine sequences everything else: the foot work happens while somebody else refills flasks.

Hot-spot protocol

The single highest-value habit in ultra foot care is stopping the moment you feel heat, rather than at the next station. A hot spot is intact skin. A blister is a wound. The gap between them is often less than a mile.

Sit down. Shoe and sock off. Dry the foot. Then find the cause before you cover the symptom, because covering a hot spot without removing the cause just means it comes back in an hour: shake grit out of the shoe and sock, check for a folded insole edge, a rolled sock seam, or a lace crossing that is biting. Then treat the skin: dry it, lube it or cover it with a rounded tape strip, and re-lace.

Draining, and where the trail-side stops

The widely used field guidance is straightforward. A small blister that is not painful and still has an intact roof is best left alone and covered. A large, tense, painful blister that is going to rupture on its own anyway is commonly drained: clean the skin with an alcohol wipe, puncture at the lowest edge with a sterile needle, press the fluid out, and leave the roof in place as a dressing, then cover it with a non-adherent pad and tape. Never peel the roof off on course; that intact layer is the best wound cover you have.

Two categories do not get handled on the trail. Blood blisters and anything deep or under a toenail go to the race medical tent. So does any sign of infection: spreading redness, pus, warmth, red streaking, or feeling unwell. Numbness that does not resolve when you loosen your laces is also a medical-tent conversation, not a tape conversation. If you are unsure at all, walk into medical; it costs a few minutes and nothing else.

The drop-bag foot kit

Build one kit, then duplicate it so an identical version sits in every drop bag and one lives with your crew. Per bag:

  • Two pairs of socks
  • A small tin of balm
  • Ten to fifteen pre-cut tape strips with rounded corners, in a labeled zip bag
  • Tincture of benzoin swabs
  • Four alcohol wipes
  • Two individually wrapped sterile needles
  • Non-adherent dressing pads and a couple of hydrocolloid squares
  • Small scissors or nail clippers
  • A bandana or microfiber square that is only ever a foot towel
  • Nitrile gloves for whoever is helping
  • An empty zip bag for wet socks

Buying these as consumables in bulk from a medical-supply source such as Brookwood Med is cheaper and better stocked than buying five boxed first-aid kits and throwing away the contents you do not use. Wet-race and hot-race branches

For a wet race, plan more changes than you think, and change immediately after the wet section instead of two stations later. Accept that shoes will be wet and prioritize drainage over waterproofing; a shoe that drains and dries beats a membrane shoe that holds water in.

For a hot race, expect more swelling. Loosen laces before your foot tells you to, put extra sodium and fluid handling in the plan (the ranges are covered in the carbs-per-hour guide), and remember that dumping ice water over your head at every station runs down into your socks. If you are icing aggressively, add a sock change.

What crew should actually do

Give the crew one instruction: at every stop where the runner sits, both shoes come off and both feet get looked at under a light. The question is not how are your feet, because the answer is always fine; it is where does it burn. The rest of the division of labor is in the crew guide.

The mistakes that keep showing up

  • Pushing a hot spot to the next aid station.
  • Racing in socks or shoes that have not done a long run.
  • Blanket-taping a whole foot and creating new edges.
  • Taping over damp skin with no primer, so the tape rolls into a ridge.
  • Popping a blister and peeling the roof off.
  • Ignoring grit because stopping feels slow.
  • Lacing to the same tension at mile 70 as at the start.

Do the boring version of all of this and most ultras become a fueling and pacing problem rather than a foot problem, which is a much better problem to have.

FAQ

Should I pop a blister during a race?

Common field guidance is to leave small, painless blisters with an intact roof alone and cover them, and to drain a large, tense, painful one that will rupture anyway using a sterile needle at the lowest edge while leaving the roof on as a dressing. Blood blisters, blisters under a toenail and anything showing redness, pus or warmth go to race medical instead. If you are unsure, walk into the medical tent.

Do toe socks actually prevent blisters?

They eliminate one specific mechanism, toe-on-toe shear, which is a common blister site in ultras. They do nothing about moisture, grit or a badly fitting shoe, so treat them as one layer of the stack alongside lube, sock changes and gaiters rather than a complete solution. Run at least one long training run in them before racing.

How often should I change socks in a 100-miler?
